If you prefer to fill them out at the office, please arrive at least 15 minutes before your appointment.
What to expect during your first visit
The initial exam involves the following:
A careful review of your medical history.
A complete set of digital x-rays to view the structure of your teeth and jaw.
A thorough cleaning and an evaluation by Dr. Waldbaum to check for decay or gum disease that you might not be able to notice yourself.
We also take the time to discuss any issues or concerns you have with your teeth and your plan for any long-term care.
Billing and Insurance
As a courtesy to our patients, we are happy to bill your insurance company for your services.
This includes:
Working directly with your insurance company to promptly file your claim and request payment to our office.
Researching your dental plan to advise you of your available benefits.
Following the ADA/AMA guidelines for coding procedures and filing insurance.
Dental insurance often does not cover all of the expenses. It is your responsibility to know what your plan covers and to pay any difference in the billed amount.

Our cancellation policy
We understand that your time is valuable and unforeseen circumstances may arise, where you are no longer able to make it to your appointment with our office. If you need to cancel your appointment, please call our office directly with at least 48 hours notice or more so that we may make accommodations that best suit you as well as our practice and staff.
