All Patients:

We ask all patients to update their Health History form each visit.

If you prefer to fill out the form at the office, please arrive at least 15 minutes prior to your appointment time.

Billing and Insurance

As a courtesy to our patients, we are happy to bill your insurance company for your services.

This includes:

  • Working directly with your insurance company to promptly file your claim and request payment to our office.

  • Researching your dental plan to advise you of your available benefits.

  • Following the ADA/AMA guidelines for coding procedures and filing insurance.

  • Dental insurance often does not cover all of the expenses. It is your responsibility to know what your plan covers and to pay any difference in the billed amount.

Our cancellation policy

We understand that your time is valuable and unforeseen circumstances may arise, where you are no longer able to make it to your appointment with our office. If you need to cancel your appointment, please call our office directly with at least 48 hours notice or more so that we may make accommodations that best suit you as well as our practice and staff.
